Doris McCartney, 71, and 69-year-old Ronald Swann were found dead at their Moorabbin home.
Glen William Nash on Thursday fronted Melbourne Magistrates Court charged with murdering the siblings between October 21 and 22 that year.
The 58-year-old was arrested after police last week offered a $1 million reward to help solve the case.
It was Nash's first time in custody and he had mental health issues, his lawyer said.
The court was also told Nash was undergoing radiotherapy treatment and on morphine.
He was remanded in custody to reappear on July 1 for a committal mention.
